Our Path Home: OUTREACH
Our Path Home Connect Outreach works to build rapport and develop relationships with people experiencing homelessness who are coping with severe mental illness or severe mental illness along with substance use issues. We prioritize helping people who are experiencing unsheltered homelessness and sleeping on the street to connect them to the supports they need to access emergency shelter, services, and housing.
Our team meets with people wherever they are, with the goal of helping them obtain permanent housing and supportive services, including counseling and substance use treatment. We first seek to build relationships to understand people’s stories, needs and goals. We then work side-by-side to connect them with the services, treatment, and benefits that can help meet their goals.
